I'll drink to that

Hot chocolate is a very nice drink, but I have never quite managed to achieve the Ultimate Hot Chocolate, the hot chocolate that hot chocolate is *supposed* to taste like. It should be rich, thick, warming, not tooooo sweet, and well chocolatey.

How to make Ultimate Hot Chocolate? Anyone?
